About this paper

Isabel Chien, Nina Deliu, Richard E. Turner, Adrian Weller, Sofia S. Villar, Niki Kilbertus

The authors review how machine learning and adaptive designs raise fairness questions in clinical trials. They explain that changing treatment assignments as evidence accumulates may favor promising treatments, but can distribute risks unevenly and affect statistical power. For ACTG076, a later estimate suggested a suitable adaptive design could have saved 11 newborns; this was counterfactual, not observed in the original trial.
