The authors map runtime faults in Model Context Protocol (MCP) servers, where implementation defects disrupt protocol-facing functionality despite successful startup. They manually code 833 fault threads from 473 maintained GitHub repositories into 11 categories, 27 subcategories, and 73 leaf types, then survey 55 developers. At least 25% recognized each subcategory; Tool Execution led at 73%, within a selected, nonrepresentative corpus.
