PhysioWorld improves ICU forecasting under shifts in drug-dosing practice by separating a dose-related prediction from the model’s prediction with covered doses zeroed, then constraining its direction using pharmacological rules. On MIMIC-IV’s sedative-intensity shift, it achieves 9.21 RMSE versus 10.28 for the strongest external baseline, while matching its JEPA backbone’s 7.95 in-distribution RMSE. The rules constrain signs, not treatment effects.
